Paradigm Customer Referral Program

Effective Date: March 3, 2026

Sponsor: Paradigm Senior Services including its affiliates and subsidiaries (“Paradigm,” “we,” “us,” or “our”). These Terms and Conditions (“Terms”) govern participation in the Paradigm Customer Referral Program (“Program”). By participating in the Program, you agree to be bound by these Terms.

1. Program Overview

The Paradigm Customer Referral Program allows eligible existing Paradigm customers (“Referrers”) to refer prospective provider organizations (“Referred Providers”) to Paradigm. If a Referred Provider becomes a qualifying paying customer under the conditions outlined herein, both the Referrer and the Referred Provider may receive a reward.

This Program is intended for legitimate business referrals only and is not intended to incentivize improper conduct or influence clinical, medical, or regulatory decisions.

2. Eligibility

2.1 Referrer Eligibility

To be eligible to participate as a Referrer:

  • You must be an active Paradigm customer in good standing at the time of referral and at the time of reward issuance.
  • Your account must not be delinquent, suspended, or in default under any agreement with Paradigm.
  • You must be authorized to participate in this Program under applicable law and your organization’s internal policies.

Paradigm reserves the right to determine eligibility in its sole discretion.

2.2 Referred Provider Eligibility

To qualify under the Program, a Referred Provider must:

  • Be a new customer to Paradigm.
  • Not be an existing Paradigm customer at the time of referral.
  • Not have been a Paradigm customer within the preceding twelve (12) months.
  • Not be actively engaged in sales discussions, contract negotiations, or implementation with Paradigm at the time of referral.
  • Execute a services agreement with Paradigm.
  • Submit claims through Paradigm and have at least one (1) claim successfully processed and paid (“First Paid Claim”) within ninety (90) days of the referral submission date.

Paradigm reserves sole discretion to determine whether a Referred Provider qualifies as “new” and eligible under the Program.

3. Referral Submission Requirements

To qualify for a reward:

  1. The referral must be submitted through the Referrer’s unique referral link or designated official referral submission mechanism provided by Paradigm.
  2. Referrals submitted via email, phone call, website inquiry forms, informal introductions, or other unofficial channels will not qualify.
  3. The referral must be submitted prior to the Referred Provider initiating contact with Paradigm independently.
  4. The Referrer must have obtained appropriate consent from the Referred Provider to share their contact information with Paradigm.

Paradigm is not responsible for incomplete, inaccurate, lost, delayed, misdirected, or improperly submitted referrals.

4. Reward Structure

4.1 Reward Amount

If all qualifying conditions are met:

  • The Referrer will receive a $100 digital gift card.
  • The Referred Provider will receive a $100 digital gift card.

No substitutions or cash equivalents will be provided unless required by law.

4.2 Triggering Event

Rewards will be issued only after:

  • The Referred Provider has executed a services agreement with Paradigm; and
  • The Referred Provider has successfully submitted claims through Paradigm; and
  • At least one claim has been successfully adjudicated and paid.

The “First Paid Claim” date shall be determined by Paradigm’s internal billing records and shall be final and binding.

4.3 Payment Timing

Rewards will be processed and distributed quarterly following verification of eligibility.

Paradigm may use a third-party rewards fulfillment provider to distribute digital gift cards. Participants may be required to provide additional information necessary for reward fulfillment and tax reporting.

Paradigm is not responsible for:

  • Incorrect email addresses,
  • Spam filtering,
  • Expired redemption links,
  • Unclaimed rewards after notification.
5. Program Limitations
  • There is no stated cap on the number of referrals a Referrer may submit; however, Paradigm reserves the right to limit or cap rewards at any time in its sole discretion.
  • Self-referrals are strictly prohibited.
  • Referrals involving shell entities, affiliated entities under common ownership, or entities controlled by the Referrer may be disqualified.
  • Rewards are non-transferable.

Paradigm reserves the right to deny any referral that it determines, in its sole discretion, does not meet the spirit or intent of the Program.

6. Compliance and Legal Considerations

Participants are solely responsible for ensuring that participation in this Program:

  • Complies with all applicable federal, state, and local laws,
  • Complies with healthcare regulatory requirements,
  • Does not violate anti-kickback, anti-inducement, or similar laws,
  • Complies with their organization’s internal compliance policies.

This Program is not intended to induce referrals of patients or services reimbursable under federal healthcare programs and should not be interpreted as such.

Paradigm makes no representation regarding the legal or tax consequences of participation. Participants are responsible for any taxes arising from rewards received.

7. Taxes

Rewards may constitute taxable income. Paradigm reserves the right to issue IRS Form 1099 or other tax documentation as required by law.

Participants are solely responsible for reporting and paying any applicable taxes.

8. Fraud, Abuse, and Misuse

Paradigm reserves the right to withhold, deny, or revoke rewards if it determines, in its sole discretion, that:

  • The Program has been abused,
  • False information was provided,
  • Artificial or bad-faith referrals were made,
  • The referral was part of a bulk lead-generation scheme,
  • There was any attempt to manipulate or circumvent Program rules.

Paradigm may suspend or terminate participation for any participant suspected of misconduct.

9. Program Modification or Termination

Paradigm reserves the right to:

  • Modify these Terms at any time,
  • Change reward amounts,
  • Add eligibility requirements,
  • Suspend or terminate the Program at any time without prior notice.

Any changes will apply prospectively and will not affect rewards already earned prior to modification.

10. No Guarantee of Sales

Participation in the Program does not:

  • Guarantee that a Referred Provider will become a customer,
  • Guarantee reward eligibility,
  • Create any partnership, joint venture, agency, or employment relationship.

Referrers may not represent themselves as agents or representatives of Paradigm.

11. Limitation of Liability

To the fullest extent permitted by law, Paradigm shall not be liable for:

  • Indirect, incidental, consequential, or special damages,
  • Lost profits,
  • Lost business opportunities,
  • Errors in reward fulfillment,
  • Program interruptions or technical failures.

Paradigm’s total liability under the Program shall not exceed the value of the reward owed.

12. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of [Insert State], without regard to conflict-of-law principles.

Any disputes arising under this Program shall be resolved exclusively in the state or federal courts located in [Insert Jurisdiction].

13. Acceptance of Terms
